• Additional costs for locations outside the above mentioned

Cancellation on Private Lessons: 2 weeks written notice

• Dates and times team/association is available

• Travel and/or Facility location

• How many lessons per week

• How many lessons per month

• Age division of players

• Association or Team

• Team experience; new, beginner, intermediate, advanced

• Indicate number of particiapants on the ice
